I have found Equicare to be a great tool for managing my case load in Navigation. The reminders help me to plan, and then follow up on my work. I can leave tasks for someone to follow up on in my absence and it's all easy to track.. I have a group of patients that need follow up at different intervals and it's easy to put reminders out even a year away. It's great.

Equicare has changed our Nurse Navigation practice from “hit and miss” to “strategic planning” for patient care. The format provides a way to track patient contacts, see future appointments and put in reminders for upcoming procedures, labs and scans. We have multiple ways of documenting in Equicare to show in real time what is happening with our patients.

The patient portal provided by Equicare has helped us connect with our patients on another level outside of the clinic. It has allowed our patients to access their daily appointment times, educational materials, and view or download their health summaries or send them to another health care provider.

With Equicare we have been able to track and report the patients that have had to reschedule due to COVID-19 concerns. This data, combined with patient feedback, has allowed us as a business to determine the markets where Telehealth solutions will make the most impact. Our patients have been grateful to continue their treatments and scheduled Provider visits during this trying time.

Equicare CS survivorship and navigation tool has really impacted our nursing staff. The Support team is there 100% of the time, is responsive and has been remarkable. They really helped transition our teams across multi-system sites, which can be difficult when trying to bring everyone live onto a new process.
